About Longleaf Care Guide
Local guidance for families facing senior-care decisions.
Longleaf Care Guide was created to make the first step feel less overwhelming. Families can share what is going on, compare care options, and be connected with an appropriate local care provider or senior-living placement resource when coverage is available.
What we do
We help families sort through in-home care, independent living, assisted living, and skilled nursing options. The goal is not to pressure anyone into a decision. The goal is to help the family understand the next reasonable step.
When a request is submitted, we review the care type, location, timing, payer source, and preferences shared by the family. If there is an appropriate match, the request may be shared with a local care provider or placement partner.
We also follow up after referrals so families are not left wondering whether anyone received their request.
Where we focus first
Longleaf Care Guide is starting with families in and around the Sandhills and surrounding communities.
Important limitation
Longleaf Care Guide is a referral and guidance resource. We do not provide care directly, make medical decisions, verify every outcome, or replace independent family evaluation of providers. For emergencies, call 911.
Free for families
Families do not pay Longleaf Care Guide. If we connect you with a participating care provider or senior-living placement partner, we may be paid by that partner.
