We collect contact details, location, requested care type, timing, payer source, contact preferences, and any care, health, mobility, or living-situation information you voluntarily provide.
How we use information
We use this information to evaluate your request, identify an appropriate care provider or senior placement adviser, facilitate contact, follow up on referral progress, improve our service, and maintain operational records.
How information is shared
Information may be shared with a provider or placement adviser reasonably matched to the requested service and coverage area. We do not intend to broadly blast your information to unrelated providers. We may also share information when required for operations, security, legal compliance, or dispute resolution.
Compensation disclosure
Families do not pay Longleaf Care Guide. If we connect you with a participating care provider or senior-living placement partner, we may be paid by that partner.
Retention and security
We retain referral records for operational, compliance, reporting, follow-up, and dispute-resolution purposes and use access controls intended to limit records to authorized administrators and assigned providers.
Your choices
You may ask us to correct or delete information, subject to legal, safety, compliance, and operational retention requirements. Do not submit information you are not authorized to share.
Important limitation
This service is a referral and guidance platform, not a medical provider, emergency service, insurer, or legal adviser. Do not use the form for emergencies. If there is immediate danger or a medical emergency, call 911.
