Referral service
Longleaf Care Guide helps families connect with independent care providers and senior placement advisers. We do not provide care, guarantee availability, endorse a particular provider, make medical decisions, or replace your independent evaluation of any provider, community, contract, price, license, or service.
Free for families and compensation
Families do not pay Longleaf Care Guide. If we connect you with a participating care provider or senior-living placement partner, we may be paid by that partner.
Your responsibilities
You agree to provide accurate information, share only information you are authorized to disclose, independently evaluate providers, and call emergency services when urgent assistance is required.
Provider independence
Providers and placement partners are independent organizations responsible for their own licensing, staffing, screening, insurance, services, pricing, contracts, recommendations, and conduct.
No guarantee
A submitted request does not guarantee a match, response time, acceptance, placement, care start date, coverage, provider suitability, or outcome.
Acceptable use
You may not misuse the service, submit fraudulent requests, interfere with security, or access information you are not authorized to view.
Not for emergencies
Longleaf Care Guide is not an emergency service, medical provider, insurer, legal adviser, or crisis line. If there is immediate danger or a medical emergency, call 911.
Changes
We may update these terms as the service develops. Continued use after an update constitutes acceptance of the revised terms.
