Many families are already overwhelmed before they fill out a form. Longleaf Care Guide is built to make the process feel calmer, clearer, and more local.

We start with the family’s actual situation

A request can include the type of care being considered, location, timing, payer source, preferred contact method, and what has changed at home. That helps us understand what kind of next step may fit.

We do not aim to send requests everywhere

The goal is thoughtful routing, not volume. When possible, we connect a family with an appropriate care provider or placement partner based on the request and local coverage.

We separate different care paths

In-home care and senior-living placement are different needs. Longleaf Care Guide routes those requests differently so families are not treated like every care question has the same answer.

We use local service-area matching

Coverage matters. We consider county and ZIP service areas so a family is not routed to a provider who does not serve their area.

We include follow-up

After a referral, we can check whether contact happened and whether the family needs another next step. Families should not be left wondering where the request went.

We keep the cost clear

Families do not pay Longleaf Care Guide. If we connect you with a participating care provider or senior-living placement partner, we may be paid by that partner.

Being clear about limits is part of earning trust. A referral is a starting point, not a guarantee.

  • We are not an emergency service. If there is immediate danger or a medical emergency, call 911.
